By booking an appointment you agree with the following terms:

  1. Family Members: Add Family Members to your account and book a separate appointment for each person. Ensure each person has completed their intake form.
  2. Intake Forms: Please complete the Intake Form that will be emailed to you. Test results cannot be shared if Intake Form is not complete.
  3. Disclaimers: By booking the appointment, you are agreeing to be solely responsible for determining the correct date, time, and test type for your travel needs. You are also agreeing to our Terms and Conditions of your Appointment and have read our Frequently Asked Questions for more information.
  4. Cancellations: Cancellation/rescheduling within 2 days of your appointment, or if missed appointment, will result in a charge of $75 for PCR/NEAR and 50% for Antigen. For mobile testing, the concierge fee will be forfeited in case of last-minute cancellation.
  5. Refunds: $10 admin fee applied per refund processing. To use the credit on file for another appointment, please contact us at support@bonvoyagemedical.com
  6. Results: If you have any questions about your test result, please email results@bonvoyagemedical.com with “URGENT!” in the subject line or call us at 604-200-1029 and follow the prompts.
  7. Customizations: Any certificates (e.g., Japan Medical Certificate) will be charged at $75 + GST per page.
