• INTAKE FORM: Intake forms MUST be completed prior to having your appointment.
• CANCELLATION: Cancellations within 48 hours will incur a $50 cancellation fee. If you prefer not to keep the remaining balance on file as credit please email us to request a refund at hello@bonneviemedical.com.
• IDENTIFICATION: You MUST bring a piece of government issued photo ID to your appointment. If travelling, please bring your passport, or a clear digital picture of it.
